All students enrolled in our program are required to have proof of health insurance valid in the United States. Your proof of insurance must include these minimum requirements:
- Name of student
- Dates of coverage
- Sickness Minimum coverage of $50,000 USD per incident
- Injury Minimum coverage of $50,000 USD per incident
- In English
- U.S. contact information listed
The Tutorium highly recommends health insurance for F-1 students in the United States (not citizens or residents) that includes:
- Repatriation ($7,800 USD minimum)
- Expenses associated with medical evacuation ($10,000 USD minimum)
The Tutorium does not offer health insurance, but you can use this list of insurance providers that we recommend.
If you have dependents (family) with you, you should buy insurance for them as well as yourself. Health care costs for your family could negatively affect your ability to study.
You should have dental and vision check ups before you leave your home country. Vision (eye) care, dental (teeth), hearing (ear) care are usually not covered in health insurance. UIC Required Vaccinations Heading link
All students regardless of starting term and number of credit hours registered are required to receive the COVID Vaccine and Booster.
All incoming students born on or after January 1, 1957 who are enrolled for 6 or more credit hours must present documented proof of immunity (pdf) against the following diseases:
- Measles (Rubeola) – two (2) doses at least 28 days apart
- Rubella (German Measles) – two (2) doses at least 28 days apart
- Mumps – two (2) doses at least 28 days apart
TD (Tetanus and Diphtheria) – at least one (1) dose within past 10 years
- Meningitis – Required for students under the age of 22 – Must be given on or after 16th birthday

UIC COVID-19 Saliva Test Procedures
UIC provides free COVID-19 testing (PCR tests) for all students, faculty and staff. Instructions for taking the test: https://today.uic.edu/covid-19-saliva-testing
Campus COVID-19 testing sites are located at:
- Student Center East, 750 S Halsted St.
- Student Center West, 828 S. Wolcott Ave.
Monday-Friday, 9 a.m.-5 p.m.
Saturday, 9 a.m.-1 p.m.
Starting Sunday, Jan. 23, Student Center East is also open:
- Sunday, 10 a.m.-2 p.m.
Starting Monday, Jan. 24, Student Center East and Student Center West are open:
- Monday-Friday, 8 a.m.-6 p.m.
- Saturday, 9 a.m.-1 p.m.
All testing sites are walk-in only. The last person is accepted 15 minutes before closing.
Before the saliva test
New students must:

- In Self-Service, select “University of Illinois Chicago.”
- Log in using your NetID and Password.
- In the Main Menu, select “Personal Information.”
- In the Personal Information menu, select “Addresses & Phones.”
The day of your saliva test
- Do not eat, drink, brush your teeth, chew gum, use mouthwash, or use tobacco for 30 minutes before your saliva test.
- If you have any symptoms of COVID-19, you cannot take the saliva test. Be aware of the symptoms of COVID-19. If you have these, please contact your medical provider to seek care and determine if diagnostic testing is recommended.
- Bring your UIC i-card to the testing site. You will need to show it in order to be tested.
- No food or drink is allowed in the testing site.
- If you are under the age of 18, you must provide a signed consent form from your parent/guardian before testing.
Taking the test
- You will be asked to provide a saliva sample. The sample collection process takes 3-5 minutes and is painless.
- You will receive your test results through MyChart within 24-48 hours.
- If you test positive, you will be contacted by UIC with instructions.
Do NOT take the saliva test:
- If you have symptoms of COVID-19.
- If you have had a positive COVID-19 test within the past 90 days.
- If you have had a saliva test in the previous 48 hours.
You can take the saliva test for COVID-19 after being exposed to someone who has tested positive for COVID-19 on day 7 or later from last exposure.
Saliva testing is for individuals who are not displaying symptoms of COVID-19.
Saliva test results
Saliva test results are available in MyChart, the Patient Portal used by UI Health.
- If you do not activate MyChart before your testing saliva appointment, you will be asked to activate it at the testing site.
- If you already have a MyChart account through UI Health or another healthcare provider, the results will be sent to the email address you have in MyChart.
UIC Daily Pass
The UIC Daily Pass shows your current saliva testing status in addition to daily self-reported status with color codes.
- You have had at least two negative saliva tests in the past 7 days, or
- You have had a negative saliva test in the past 48 hours, or
- You are not in the saliva test program because you have been diagnosed with COVID-19 within the last 90 days**, or
- You are exempt (CLEARED) from UIC saliva testing requirements as a result of being fully vaccinated against COVID-19
- You have had a recent saliva test but the results are not available yet
- You are not cleared from mandatory testing and have not met the testing requirements needed to turn your badge GREEN or YELLOW, or
- Your most recent test was inconclusive, and you have not had a second retest within 72 hours
Students who received a COVID-19 vaccination exemption, or are only partially vaccinated, must record their health status before coming to campus each day by using UIC Healthcheck.
UIC COVID-19 Booster Requirement
How to schedule your appointment
- If you received your COVID-19 vaccine at a UI (University of Illinois) Health vaccine location, you will receive an email and notification in MyChart letting you know that you can schedule an appointment.
- You can then schedule your appointment at one of the UI vaccine locations using MyChart.
If you would like to get the COVID-19 booster but can’t schedule it in MyChart, you can call 833.536.9829 to schedule an appointment.
Providing Proof of Your Booster
- Upload your proof of booster to Healthcheck.
- If you have already submitted an initial vaccination verification, you will be prompted to submit the location, date and type of booster you received.
- After submitting and upon receiving a follow-up email, follow the steps to upload your updated vaccination card.
Exceptions to the booster requirement
- If you have previously received a vaccine exemption, it will be extended for the booster and no further action is necessary.
- If you have had COVID-19 in the last 90 days, you are not required to receive a booster shot until after 90 days from your infection.
Will UIC recognize COVID-19 vaccines being administered outside the U.S.?
UIC plans to recognize all COVID-19 vaccines in addition to the Pfizer, Moderna, and Johnson & Johnson versions being administered in the U.S. Bring evidence of your COVID-19 vaccine with you to the U.S.
If I have not yet received a COVID-19 vaccine, can I get one when I arrive on campus?
COVID-19 vaccines are currently available to all members of the UIC community , ages 12 and up. This vaccine is free.

Do I need to show COVID-19 test documentation when entering the U.S.?
Yes. All air travelers must show proof of a negative COVID-19 test taken within the previous 3 days in order to enter the U.S. Please see the CDC website for more information.
What to do if you get sick
If you experience any cold symptoms, you must:
- First, complete this COVID-19 Decision Reporting Tool
— This form will tell you if you need to fill out the COVID Intake form in step 2.
- COVID Intake Form
— This form is where you give your symptoms, tell when you were in class, and list the people you came in contact with.
- Use a computer. These forms do NOT work on a phone.
- If you need help with these forms, ask your teacher.
- Your teacher may ask you to show the email that you receive from the COVID contact tracers as proof of your illness.
- You can expect to be home at least a week.
While you are home, remember to:
- communicate with your instructors,
- attend your classes online,
- and complete your assignments.
PLEASE NOTE: This process is separate from the Healthcheck site or Daily Pass site.
The Healthcheck website, the twice weekly saliva testing, and Daily Pass are only used by unvaccinated/exempt students to gain access to campus and buildings. They are NOT for reporting sickness, receiving resources for being sick, or contact tracing.
